Welcome to the LifeSpan family. Rest assured you have made a good decision in finding the right place for your child during the times you are away.
For your convenience, you can download the LSDC enrollment package for registration in our centers. Please be sure to complete each form in its entirety. Return your completed enrollment package to the Family Coordinator who will be happy to assist you and help to determine your official start date.
We accept child care subsidies and work in conjunction with the Department of Public Welfare (Child Care Works) and Child Care Information Services. If you are in need of financial assistance to help pay for child care, please contact:
